The purpose of this form is to help you in getting your DMV Title or a Lien Release, please be advised it takes up to 21 days to receive your Title after your payoff date.



If your loan has been paid more than 21 days and the title has not been received or has been lost and you require a Lien Release there is a $20.00 fee for this service. Fee along with this request and a copy of your current registration needs to be mailed to KeyPoint Credit Union 2805 Bowers Avenue, Santa Clara, CA 95051 ATT DMV. Or if you currently have an KeyPoint Credit Union account (sav.etc..) fax this request and a copy of your current registration to 408-731-4319. And we can debit your account.
